We ask that you please complete all required forms (COVID-19 questionnaire, safety liability form and new patient registration forms - if applicable) prior to your arrival. New medical history forms must be completed by all existing patients. Forms can be accessed in the Forms tab on our website: www.uppereastdentist.com

Arrival Time - please arrive on time and not too early for your appointment to reduce waiting room traffic
Guest Policy - please come to your appointment solo, unless accompanying a child for their pediatric visit (1 parent/guardian per child, no siblings unless they have an appointment as well)

While gowns, N95 masks, and gloves are a major concern when caring for patients, they’re also in short supply, and more expensive than ever. Unfortunately, we have little to no leverage with insurance companies and they have refused to raise their reimbursement rates. In an effort to mitigate risk exposure to patients and staff and create the safest treatment environment, the American Dental Association has authorized dental offices to charge this safety fee in lieu of increasing procedure fees. --- need help with this wording
